I am requesting disclosure of non identifiable, policy level information regarding foster care provision within Telford & Wrekin. For the avoidance of doubt, I am not seeking any personal data relating to individual foster carers, in accordance with the Data Protection Act 2018, UK GDPR, and the confidentiality requirements under the Children Act 1989. Please provide the following information: 1. The total number of approved foster carers within Telford & Wrekin, broken down by approval category (e.g., mainstream, connected persons, respite, emergency, therapeutic). 2. A summary of the qualification and training standards required for foster carers, including mandatory training, specialist modules, and any therapeutic competencies. 3. Details of the therapeutic fostering provision commissioned or delivered by the Local Authority, including: •the therapeutic model(s) used, • accreditation requirements, and • oversight or quality assurance arrangements. 4. Any current quality assurance or regulatory frameworks governing foster carer training, supervision, and therapeutic practice within the authority.

Having searched for this information previously, and not having found it, I question whether my foi request is articulated sufficiently. I am not asking for information about the phso- police relationship generally. My question concerns phso obligations to complainants, to the state, where authorities such as the police fail in their prescribed duty so, prejudicing the complainant, perhaps putting the complainant or patient at risk. Its understood the phso remit is one very much limited to offering recommendations however, im quite aware you hold scope to escalate to government, the gmc etc - police not being listed here. Surely where there is serious allegation, one of these authorities may be better placed than the phso to deal with such matters. Does there exist policy here - in relation to alarming authority better placed than you to handle matters outside your remit.

In response to your very unhelpful response: • I have complained to Ofgem about the Energy Ombudsman, and they have failed to deal with my complaint regarding their performance. • I have complained to my member of Parliament which has gone nowhere • I have complained to the Department of Energy Security & Net Zero who refer me back to Ofgem • I have complained to the Public Accounts Committee without response • I have even contacted Rupert Lowe, the only member of the PAC who seems to be awake with no response so far. • I contacted the Cabinet Office who referred me to you. You seem to infer that I have not exhausted all avenues. As you have refused my request without reasonable excuse, I require a review of your handling of my request, which was for an email address in order to make a complaint following the process on the Government Website. It is noted that you failed to append a name to your response.

What steps does it take to identify correct authority? The PHSO identifies also, the complaint a matter for police. What are its obligations where the complainant identifies failure of the police to act appropriately.

I am writing to request information under the Freedom of Information Act 2000. For the years 2023, 2024, 2025, and 2026 to date, please provide the following information: 1. The total number of service users or carers which filed complaints through PALS against ambulance staff for any misconduct whilst under their care, classified by job title of the profession. 2. The total number of service users or carers which filed complaints against ambulance staff who committed acts of alleged sexual offences; and/or serious misconduct allegations that had a sexual nature whilst in their care. 3. For each complaint identified above, please provide the outcome of the complaint e.g. dismissal/payout without identifying anyone involved. 4. If the data is held, please provide an aggregate total of the financial payouts received by service users for each classification of sexual offence complaints. If the data is held, please break down this data by year and month. Please also provide an aggregated total for the years 2023 to 2026, inclusive.

I am grateful for the material you pointed me to, but I need clarification to ensure the response is complete and auditable given the safeguarding and systemic risk focus of my request. Please provide the following in writing within 10 working days: 1. Search scope — list the teams and document repositories searched (for example: Service Model team, Legal, Information Governance, Safeguarding lead, Training/HR, SharePoint, case management systems) and the dates those searches were carried out. 2. Section 21 pointers — for items you say are “reasonably accessible,” please confirm the exact URLs or document titles you rely on under Section 21 so I can be sure I have reviewed the same material. 3. Materials held under other titles — if DPIAs, training materials, or guidance on acting without full local resolution exist within broader documents, identify the document titles and provide copies or links (redacted only where strictly necessary). 4. DPIA confirmation — if safeguarding related DPIAs are incorporated into wider DPIAs (for example, a DPIA for complaint handling or casework systems), please provide the DPIA title and a copy or link. 5. Training records — confirm whether searches included training/HR systems and, if training exists on Duty of Candour, safeguarding, or systemic patient safety failures, provide the titles and a short description of the content. 6. I would be grateful if you could review the attached correspondence from the PHSO in 2019 and let (redacted) know if any of the circumstances around the PHSO competencies have changed, or indeed the success rates of the PHSO in making non-binding judgements against ICBs in similar cases.

The Service has "decided to focus on the more serious complaints". Whilst I can understand the reasons for that to occur going forward, you have not provided any explanation about historical cases. Please can you advise when the decision was made by the Service, and when the change in focus came into effect? Where can the change in service be publicly viewed and what are the terms for cases that were in progress prior to the decision being made?
