This is a request under FOIA for details of instances in which The Parliamentary and Health Service Ombudsman (including, where applicable, any agencies, bodies, or units for which it is ultimately responsible for supplier payments) has failed to pay its suppliers on time, thereby causing liabilities for late payment compensation and/or interest on late payments to be incurred for the public purse. With the above points in mind, we request under FOIA that you provide, (preferably in Microsoft Excel or an equivalent machine-readable format) the following information in respect of suppliers which were not paid in within 30 days for the period starting 1 April 2023 to the date of this request: 1. Supplier Name 2. Invoice Date 3. Gross Invoice Value 4. Payment Date 5. Late Payment Compensation or Interest Paid (if any)
Please provide the policies from which staff work from at PHSO on complaints ( as in how they may be submitted) and when receiving a complaint (the expectations of staff when receiving a complaint).

I am writing in regards to the email pasted to you. I contacted the hospital at Durham and have heard nothing of the enquiry. I have also asked for the disclosure of the staff and I was ignored. I am writing to you as I believe the hospital not only are withholding the information they have also withheld the inquiry process. This happened on the 9th of may at the outpatients appointment for my grandson (Redacted) I have yet to know the disciplinary outcome of the incident and I have the right to the names of the staff via the 2000 freedom to information act.
1. All PHSO internal policy or guidance on: o Handling complaints involving Articles 2, 3, 6, 8, 13, and 14 of the Human Rights Act 1998; o Handling referrals containing statutory safeguarding disclosures; o Accepting complaints from individuals who request reasonable adjustments under the Equality Act 2010; o Triage and escalation criteria when complaints are addressed to the Chief Executive; 2. The date my complaint submitted on 5 May 2025 was formally: o Logged, assigned, or rejected; o Or otherwise categorised or referred internally; 3. Whether the 8 June 2025 escalation to (redacted) and others was: o Registered as a continuation or new complaint; o Marked as involving safeguarding or public interest disclosure; 4. Any internal legal advice, policy guidance, or decision-making documents confirming that PHSO must accept public interest disclosures or safeguarding referrals regardless of format, in line with: o R v Home Secretary ex p Doody [1994] 1 AC 531 o R (Anufrijeva) v Southwark LBC [2003] EWCA Civ 1406 o R (Balajigari) v SSHD [2019] EWCA Civ 673 o R (Jeyeanthan) v SSHD [2000] EWCA Civ 44; o Or equivalent public law principles and administrative fairness obligations.
However, by far the majority of NHS cases are 'closed after initial enquiries'. How many of the cases fall into this category as listed on page 45, or which headings would fall under this category if that is easier to address please?

Dear Parliamentary and Health Service Ombudsman, Please provide complaint info (complaints made, rejected, partially upheld, upheld) for CAFCASS over the past 15 years.
I am writing to submit a formal Freedom of Information Act 2000 request regarding complaints received by your office concerning the Home Office and its handling of serious immigration-related matters. I would be grateful if you could provide the following information via email: 1. Complaints About Home Office Investigations • The number of complaints received from individuals stating that the Home Office failed to investigate concerns they reported (e.g. immigration fraud, criminal activity, marriage abuse, overstayers, etc.). • A breakdown by year, for the last full 5 years (or if possible, for the last 10 years ). 2. Marriage Fraud / Marriage Abuse Complaints • The number of people who have filed complaints or reports concerning marriage fraud (i.e. where a partner allegedly used them for a visa/passport). • How many victims specifically raised concerns that they were being used for immigration purposes, and/or that their partner became abusive, threatening, or dangerous. • The number of complaints made regarding the Home Office’s failure to act or properly investigate these marriage abuse cases. 3. Immigration Crime Reporting Portal • The number of complaints received regarding the Home Office Immigration Crime Reporting Tool (online portal). • Specific complaints stating that: • The portal does not work or has technical issues. • Individuals did not receive confirmation or follow-up updates after reporting concerns through the tool. • Reports made through the portal were ignored or unresolved. 4. General Home Office Complaint Handling Failures • How many individuals submitted complaints about the Home Office Complaints Department not responding to their concerns (e.g. complaints sent to complaints@homeoffice.gov.uk or similar channels). • How many complainants reported no acknowledgment or follow-up after submitting formal complaints. 5. Maladministration and Impact on Victims • The number of complaints that mention maladministration by the Home Office in immigration cases. • Data or case outcomes showing how these failures have negatively affected people’s lives, such as: • Loss of housing, income, legal status • Mental or physical health deterioration • Risk to personal safety • If available, summaries or anonymised outcome reports relating to those complaints. 6. Systemic Issues • Any statistics or internal assessments indicating that the Home Office systematically fails to take immigration fraud, marriage abuse, or domestic safety concerns seriously, particularly when reported by victims. • Any investigation findings, published reports, or internal reviews relating to these issues. Systemic Issue Recognition • Has the Ombudsman identified or reported systemic failings by the Home Office in dealing with: • Victims of immigration-related abuse • Marriage fraud complaints • Improper use of GDPR to justify non-engagement • Failure to act on safeguarding or criminal reports • If so, please provide access to or a copy of any relevant reports, recommendations, or decisions made. I also refer to the 2014 report by the Parliamentary and Health Service Ombudsman titled: “Home Office failures put a family in danger” (A 2014, report into a complaint by Mrs A and her family about the Home Office) This report exposed serious failings by the Home Office, including inadequate safeguarding measures, poor communication, and delays in responding to critical concerns — all of which left a family at risk. I am requesting information to understand how many similar complaints have been made since that case, whether these types of failures are still occurring, and if any patterns of systemic failure have been identified by your office in the years that followed. 7. Complaints About the Home Office Failing to Follow Its Own Policies • How many complaints have you received from individuals stating that the Home Office failed to follow its own published policies, procedures, or internal guidelines? • Where possible, please provide: • A breakdown by year (from 2014 to present) • The nature of the policies alleged to have been breached (e.g. safeguarding, victim protection, immigration enforcement) • The outcome of those complaints, including any findings by your office that confirmed policy breaches or maladministration. Any recent Performance statistics, would be helpful especially anything showing the compliance with recommendations and the satisfaction with the PHSO process if this could be show with demographics that would be excellent.
First, I would like to request the same information for the year 2024-25 - when it becomes available (I appreciate that it may not be currently available). Second, I would like to request information concerning service improvements issued by the PHSO during 2024-25. I would also like to request information concerning the generic types of service improvement recommendations (eg an action plan, staff training) broken down between health and parliamentary complaints - when this information becomes available (I appreciate that it may not be currently available).

Thank you for the partial response and I apologise for my delayed reply. Unfortunately only one part of my four-part request was answered and there seems to have been a misunderstanding because use of the logo to signpost people to the PHSO is not relevant to my request. Nor did I ask for data about any individuals or for the PHSO Disciplinary Policy. I gave several names only as keywords to assist in locating any correspondence. To recap, please see the unanswered points of my original request below: Please could you provide: 1. Any policy about permission for public endorsements by the PHSO and the permitted use of the PHSO logo by external organisations indicating that PHSO is a "Supporter" or similar. 2. (This was answered - thank you). 3. All data which refers to WhistleblowersUK, WBUK and/or the All Party Parliamentary Group (APPG) on Whistleblowingand/or Office of the Whistleblower and/or The Whistleblowing Bill. I would prefer the search to cover the time period from March 2018 but if that would exceed the relevant cost limits, please could you adjust the timespan to make it achievable up to the present date? 4. The item which I seek in particular is anything which refers to consent being requested, granted or withdrawn about allowing WBUK and/or the APPG on Whistleblowing to use the PHSO logo on their websites or other material, including soliciting donations.

“I am writing with regards to the Earl Report which was laid before Parliament in 2022. The Earl’s are now my constituents and I have been working with them to try and find a resolution to the issue of non-compliance from the Environment Agency. I understand Mr [redacted] was the caseworker on this case so I was hoping he could answer a question related to the outcome. In my communications with the EA, they have informed me that ‘Our calculations used to arrive at these figures were fully explained to the independent reviewers and to the Ombudsman. The Ombudsman decided to refer the matter to the Public Administration and Constitutional Affairs Committee (PACAC).’ The Earl's, however, claim that neither they nor members of the PHSO were presented with the calculations used to reach the current level of compensation – to your knowledge is this the case? The Earl’s would like to see the methodology behind the Environment Agency’s final figure to ascertain why it differed so substantially from that recommended by the PHSO. If you are able to provide any information, I would be incredibly grateful.”

How many complaints about Northampton General Hospital since 2022 were rejected as ‘not ready’ despite evidence of institutional bias? Release all internal memos discussing Northampton General Hospital complaints since 2020 to prove procedural bias.
