I would like to know how many complaints the Student Loans Company has had and whether there has been an Increase. Would it be possible to get information for the last 5 years?
Dear Parliamentary and Health Service Ombudsman, What is the normal lead time to allocate a case to a signed document. Some cases take at least six months, if one has complained before. Under what circumstances can the Ombudsman refuse to deal with a case when it has been put in front of him, or answer questions.
I am writing to request an internal review of the response I received to my Freedom of Information request (Reference: 00002983), dated 27 February 2025. While I appreciate the information provided, I believe there may be a misunderstanding regarding the scope of my request. My original request sought data on complaints investigated by the PHSO concerning His Majesty's Courts and Tribunals Service (HMCTS) during the period [Jan 2022] to [Dec 2023]. I understand the PHSO's response interpreted this as referring only to detailed investigations that led to a formal report. However, I was also interested in complaints that reached the stage of being assigned a caseworker, after the PHSO has confirmed the complainant has completed the internal complaints process with HMCTS. This caseworker was assigned to "take a closer look" at the complaint, but in some cases, a detailed investigation may not have been conducted. My intention was to understand the volume of complaints that were considered serious enough to warrant assessment by a caseworker, following confirmation that HMCTS internal processes were exhausted, even if they did not all ultimately progress to a full, detailed investigation. Specifically, I'm interested in data on cases where a caseworker determined whether the complaint warranted escalation to a detailed investigation after the initial review. To clarify my request, I would like to know if the PHSO holds data on the number of complaints that reached the caseworker stage, and the outcomes of that stage (e.g., escalated to detailed investigation, not escalated, etc.) during the specified period (Jan 2022 - Dec 2023). If my initial request was overly broad, I'm happy to refine it to focus on the specific data the PHSO holds regarding complaints that reach the caseworker assignment stage, excluding those which have not previously been through the HMCTS internal complaints process. For example, a number of complaints at caseworker stage and their outcome ( e.g. what % lead to detailed investigations) would be of significant interest. I believe this clarification will ensure that the information provided accurately reflects the scope of my original request. Thank you for considering my request for an internal review. The total income your organisation received from the government in each year. 2. Your organisation’s total expenditure in each year. 3. Your organisation’s total staff headcount in each year. 4. The year in which your organisation was originally founded For the purposes of this FOI, income from the government refers to all income you received from the UK government.. This is most likely in the form of government grants but could also be through other mechanisms. Total expenditure refers to the total amount of money your organisation spent in each financial year. If this is not available, please provide net expenditure and identify it as such. Your organisation’s headcount refers to the total number of staff employed during each financial year. If a total headcount is not available please provide the full-time equivalent figure and identify it as such. The year in which your organisation was founded refers to the initial founding of the organisation rather than a name change. Please provide the earliest possible date for this.

I have noticed that in the Information Commissioner's Annual Report it states that 100% of PHSO decisions against the IC were dismissed. That figure is very alarming and warrants further investigation. I hereby make a Freedom of Information Request for all information pertinent to PHSO decisions concerning the IC. This includes guidance, any internal Policy, statistics, Code of Practice or any other documents that relates to reasons for your decisions.

I dont think you have responded to my email. If you have, please send it to me again. I'm finding it difficult to chase up via FOI access who seem to be out of sync with FOI requests for SAR for personal information and phone blocking actions and reviews. Im very disturbed that PHSO has returned to pre-diospositions and assumptions made by staff unknown, when this was identified by Chief Mellor some years ago as being maladministration, and that PHSO should only be working with Facts of a case. You have limited my access,so please would you give me PHSO email for reporting complaints of maladministartion and also for reporting complaints of harm caused to complaiants.
I am making a request for information on the following; 1.Joint working LGSCO/PHSO I have seen the Joint Data Controller Agreement Between Parliamentary and Health Service Ombudsman and the Local Government and Social Care Ombudsman for complaint handling carried out by the Joint Working Team, March 2023. https://www.ombudsman.org.uk/sites/default/files/Final Joint data controller agreement - LGSCO and PHSO 15 Mar 23_signed_redacted.pdf And had expected that, by now, joint complaint handling had been established. Which has raised a question about how the LGSCO had handled my complaint. Ref: (redacted), not upheld 27 Sept 2024 , and Review Request, not upheld 20 Nov 2024.Which had raised the wider aspect of the impacts of planning harm on the public’s health. And the potential impact on the NHS. But had not been considered for a joint investigation. Extract “Para 3 Joint investigations by the PHSO and LGSCO covered by this agreement are about individuals who believe they have been let down by local authorities, private adult social care providers and NHS services. This agreement does not extend to joint PHSO and LGSCO complaints that involve investigating government departments and their agencies.” I have reached the end of the LGSCO Process . but my MP did refer my case to PHSO , PHSO Ref: (redacted) . This was rejected as my complaint was not about an organisation that they could investigate. I do feel very let down by my experience of the Complaints process over the last 2 + years. I would be grateful if you were able to clarify; 1a) Has funding for Joint Working been obtained?If so, when? 1b) Whether any joint investigations have taken place since Mar 2023? 1c) If so, how many investigations have been into complaints about failure of LPAs/ Enforcement Officers to minimise the impact of construction site activities , close to residential? And the added impact on health and amenity caused by delays in responding to Complaints from their citizens. 2. My understanding is that one of the aims of the Levelling Up and Regeneration Act October 26, 2023 is to hold developers to account which our Local Planning Authority LPA failed to do during the construction phase. Whilst this was not in force during the construction phase which effectively ended Sept. 2023, all Government Depts whether Local or National bodies are now required to consider Clean Air in Planning applications. And Planning Applications should follow the guidance in PM 2.5 Targets: Interim Planning Guidance These targets require a different approach to that used by applicants and Local Authorities in response to existing air quality legislation. The new approach moves away from a requirement to assess solely whether a scheme is likely to lead to an exceedance of a legal limit and instead ensures that appropriate mitigation measures are implemented from the design stage, streamlining the process for planning and ensuring the minimum amount of pollution is emitted and that exposure is minimised. However the Clean Air Strategy already required all English local authorities (including county councils), the Environment Agency, and designated relevant public authorities, to have regard to this strategy when exercising functions of a public nature that could affect the quality of air. As does the Environmental Improvement Plan. 2a) As the Parliamentary Commissioner for Administration is responsible for both LGSCO and PHSO. Can you confirm whether the 2 organisations for which she is responsible were designated as below , and are required to comply with the Clean Air Strategy 2017, which already required all English local authorities (including county councils), the Environment Agency, and designated relevant public authorities, to have regard to this strategy when exercising functions of a public nature that could affect the quality of air. As does the Environmental Improvement Plan? Our community remains at imminent risk from LPA decisions on planning applications, already approved /in the planning process. Introducing more and more road traffic to our area. We live alongside and downwind of 2 major industrial sites, and downwind of the air and noise pollution of these sites and J1 M18, A631 SRN. Accepted by National Highways as being over capacity. I hope that you will be able to provide this information.

I am writing to you to ask for information under FOI Act 2000 . Is it the case that Trust's and the PHSO are able to look at complaints that have exceeded the 12 months time limit where it considers that it is reasonable to do so. Can you tell me the grounds for investigating complaints that were send to you after 1 year and how many did the PHSO look at over the last two years that were more than 12 months old. what is the law in this area that allows Trust or yourself to look at complaints that were made 1 year after the incident .
