"You asked whether there would be anyone other else than the case worker reviewing it. you understood that there was another team who independently reviewed the challenge. The case worker explained that we do have another team which do carry our review requests of our decision, but that not all requests go to this team. If the case worker can answer the challenge, then it would be for them respond further. It depends on the challenge made and the evidence raised, or pointing out which evidence had not been considered (for example). You said that our processes seemed to have changed and thought that all challenges get reviewed by other people. The case worker believed that that used to be the case but that had changed in the last 4 or 5 years. You asked specifically how long since this had changed as it was not clear from the website.”

I must respectfully challenge your assertion that the Parliamentary and Health Service Ombudsman (PHSO) holds no information or responsibility regarding the reporting or escalation of corruption, misconduct, or cover-ups involving statutory regulatory and public bodies such as the Legal Ombudsman, the Solicitors Regulation Authority (SRA), Hampshire Police, or the Independent Office for Police Conduct (IOPC). Under Section 5(1) of the Parliamentary Commissioner Act 1967, the PHSO has jurisdiction to investigate complaints of maladministration by government departments and certain public authorities. Each of the organisations listed above operates under statutory authority, derives public power, and performs public functions, thereby placing them within the scope of administrative oversight. When credible evidence of corruption or systemic failure exists, your office has at minimum an advisory and escalation obligation to ensure the matter is referred appropriately. Furthermore, under Article 13 of the European Convention on Human Rights, every person whose rights and freedoms are violated shall have an effective remedy before a national authority. Refusing to hold or provide policy or procedural guidance on how citizens may report institutional corruption effectively denies access to such a remedy. I would therefore request that an internal review be conducted to establish: 1. Whether the PHSO holds any internal guidance, referral frameworks, or memoranda of understanding relating to cooperation or communication with other oversight or regulatory bodies (including but not limited to the ICO, Home Office, or Ministry of Justice). 2. Whether the PHSO has, in practice, ever received or referred complaints of corruption, misconduct, or maladministration involving statutory regulatory bodies. 3. Whether any correspondence exists between the PHSO and the Cabinet Office, the Ministry of Justice, or other oversight entities regarding limitations or scope of jurisdiction concerning regulatory and complaints bodies. If, after proper review, your position remains that no such information is held, I request a detailed explanation of how this aligns with the PHSO’s public accountability duties under the Ombudsman Act 1967, the Human Rights Act 1998, and the UK’s international obligations to uphold the rule of law and ensure access to justice. Please confirm receipt of this internal review request.

I am writing to formally complain about the handling of my Freedom of Information (FOI) requests, references 00003682 and 00003361, relating to my ongoing PHSO complaint). I believe there have been significant failures in complying with the Freedom of Information Act 2000, as follows: 1. Regarding Request 00003361 You cited Section 44(1)(a) of the FOIA, claiming a statutory prohibition on disclosure. However, this exemption appears to have been applied as a blanket refusal, without evidence that: · The public interest in disclosure was considered. · Severability of the requested information was assessed. · Your duty under Section 16 of the FOIA to provide advice and assistance—such as offering summaries, statistics, or redacted versions—was fulfilled. 2. Regarding Request 00003682 You refused disclosure under Section 21, stating the information is “reasonably accessible” elsewhere. However, no specific URLs or references were provided, making it impossible for me to locate the information. Without such details, the information cannot be considered reasonably accessible as defined by the Act. These responses suggest a failure to uphold your statutory duties under the FOIA and undermine the principles of transparency and accountability. This is particularly concerning given the PHSO’s role as an independent complaints body and recent issues such as the HMCTS evidence-loss bug, which highlight the need for robust oversight. I request a full internal review of both responses. If these issues are not resolved, I will escalate the matter to the Information Commissioner’s Office and raise it with my MP, Dan Carden, for further parliamentary action.

Reason I would like to know the number of cases fully investigated and partially or fully upheld beyond the case that I have quoted, where the complaint was about the DVLA and a medical issue. Further, if the situation has changed since the PHSO reply on 24 January 2025, quoted below: “We can confirm that there have been no complaints closed as upheld/partly upheld following detailed investigation whereby the issue complained about involved a medical issue. As explained, these are the cases closed since that date. Cases that were received after that date but still in progress will not have a decision recorded against them yet therefore, we are unable to say whether these will be upheld / partly upheld. We have not included any receiving detailed investigation we have only included those that did progress to detailed investigation and are now closed.”

I Would like to request further Information regarding an Incident Which had been Reported (incident 743) to the Leicestershire Constabulary on the 29th of July 2025 that lasted for 1 hour and in which the Victim Requested the Assistance from the Constabulary throughout the Duration and no response teams attended Whilst the Incident had been reported 4 times During the Ordeal on the 29th July 2025. I Would like to request and Provide further Details regarding this Incident and Several Incidents like these as the Unlawful Abduction & Removal by forceful and threatening means without ANY Cause or Disturbance , and The removal of Innocent Citizens from their Homes Cannot simply be Forgotten or Continue as this Activity is Extremely Similar to War Crimes in England in Which People begin to simply go missing from their Homes and Should not be taken Lightly in today’s Democratic British Society. I would like to high-light an IMPORTANT need to Investigate Unlawful Incidents like these Cases which have taken Place with Innocent British Citizens who are Law abiding in the UK as these types of Cases are all to often Covered up after having been Reported with too many Authorities & Public Services without Lawful Investigation to Prevent these Cases from just becoming Social Corruptions and to high-light the Leicester Case on the 29th of July as Just one Example of this Type of Unlawful Removal of British Citizens from their own Homes for No rightful Reason as this Type of Activity Cannot be Condoned in a Lawful Society in the UK . And I Would like to Request Your Legal Guidance for Assistance with finding Legal Representation who are able to Provide Legal Representation & Guidance regarding the Deprivation of Liberty Protections as the Present British Laws need to be reviewed by reliable Legislative & Independent Committees who have the Genuine Best Interest of the British People at Heart I Would like to make this Request for further Information regarding this Incident under the Freedom of Information rights
